Lead Capture Form

Is there anywhere I can learn how to use / set up this form? I've tried using it, and it seems rather simple, but when I check the website, the box is there etc. with all types of warnings and failure items written on it ??

Hi Pramod,

I have no knowledge about mysql. but when I go to the lead capture form to create it, I have many warnings about mysql query' etc.
Am I suppose to activate mysql in my hosting account? Do I need this to use the lead capture form?

I don't understand that, I've tried it both ending with wp-admin and also wp-login.php
both the user name and the password let me in .. (?)
