These two ( "Custom Contact Forms" and Front page slider / carousel) are completely unrelated as form does not appear on front page. It is a bug in your theme. FIX it!
Hello, There is not a problem in the theme, Sometimes plugin conflicts with the theme, so you have to find any alternate plugin which can do the same task and which doesn't created the problem with the theme.
