Problem Has Not Been Fixed - Upload Error, Please check your upload size and type.

I contacted [email protected] a week and a half ago and still no word as to why my website will not upload from a computer or smartphone.
I am losing subscribers by the day. Please help ASAP
When my subscribers and I try and upload a video from our computers or smart phones , an error comes up below the upload bar saying (Upload Error, Please check your upload size and type). No matter the size of the file or type of file I try and upload this error happens after the file has been uploading for minutes. I contacted my website host provider and they said that it is nothing on their end its the website. Please help asap and see the attachment. Thank you
