Is there a way to add fields in the salesjunction check out area?

Hi,

I wanted to inquire if it is possible to add a field/s in the salesjuntion easy download checkout area?

I would like to request some info like "company name" & "phone number" upon checking out,
is this possible and if so, what instructions can you provide to accomplish this
