Gravity Forms 'list' field not working properly

This very recently worked for me, but I discovered today that the 'add a line' feature on the list field is no longer displaying (was a + button at the end of the first row). Contacted the folks at Gravity Forms and tested - it is definitely a theme conflict. Not sure if Gravity forms or Blackbird Pro had an update, regardless, I really need this fixed ASAP!!

The issue is related to form not to theme.
By the way, please provide the URL of that page where you are using that form which help us to provide you a better solution.
