Can't get form to work

I need some help to get the form working. I followed the instructions in the PDF, but I now have a form on my page and I don't know how to edit it or get it working.

Strangely, it doesn't look like the form I setup with FormGet. And Lead capture doesn't allow me to change anything within WordPress, so it's not working?

I attached a screenshot, maybe you guys can see what is happening.

I have got your concern and would like to inform you that, the theme's CSS actually overrides the stylings of FormGet form.

We can resolve this from your site.
