Adding 'Price' and 'Message' fields back in widget

Hi,

Lesson learned, but I deleted the Price and Message fields in the widget, rather than hiding it, and now the client wants it back.
Can you provide the codes for me and the place to put them? or better yet replace it for me?

Also, how do i make all of these fields 'required'?

Download a fresh theme and replace that file with new themes file which you edited previously.
It has the complete code.
