Thank you for the update on lead capture but the new plugin still does not meet my needs. I need to be able to configure you so I can create a form that captures this data on this form - perhaps it could be configured by CSS to be two columns . http://ssdihelptoday.com/ If not that then maybe if could capture Name and Email and then when the user clicks the submit button it would take the them to ssdihelptoday to complete there. Ive been trying to make this work for a long time - your prompt help is appreciated. I actually have an open case since August but was unable to allow you access to the Dashboard but I can now and I have sent it to your email from before.

Actually - you cannot use that many fields - it will not work - you can increase the size of the form but it pushes all the content down and leaves blank areas . I would like to make the form in two columns and include all the information on this form here www.ssdihelptoday.com please help
If you actually look at it right now - it only goes to 10 questions - and these two questions were not able to be added Do You Have a Social Security Attorney or Advocate? Yes No Briefly describe your medical condition: And as you can see if you use all 10 it its totally pushes all the other content out of line and messes up site www.ssdihelpcenter.com
Hello, You can display the long form with scrolling feature, for that, you need to paste the below custom CSS code in Dahshboard->Appearance->Theme Options->Styling settings->Custom CSS field.
